Community Health Representatives (CHR)

The Community Health Representatives (CHR’s) provide a wide range of support services on health education, prevention and promotion.

The CHR’s provide assistance and support to the entire health team and act as a liaison to the community and provide support services to community members.

The Community Health Representative (CHR) program is aimed at taking a supportive role in health promotion, prevention, and health education activities. This is accomplished through clinics, workshops and activities which ensure aboriginal cultural values are maintained. The main role is to motivate community members to participate in health programs and to assume active responsibility for improving and maintaining optimum level of health and contribute to improved community health conditions.

Programs and Services Provided to On-Reserve Members Include:

  •     Counselling Services,
  •     Emergency Contact,
  •     Home Visits,
  •     Family Support,
  •     Meals On Wheels Program (Elders),
  •     Breakfast Program (at school),
  •     Assisting the Community Health Nurse,
  •     Traditional camps and teachings.
